Supports coordination

Supports coordination assists individuals with disabilities or chronic conditions access and coordinate various services and supports they need. The service includes developing personalized plans that address individual's unique needs and goals and connecting them to resources that can provide them with the highest quality of care and support in an efficient, effective, and cost-effective manner.

To be eligible to receive IDD Supports Coordination in Pennsylvania, individuals must:

• Have an IQ score of 70 or below

• Be a legal resident of Pennsylvania

• Have significant limitations in at least three areas of daily living
• Records indicating that you had an eligible intellectual disability before the 22nd birthday
